SHOW Location:  MONTGOMERY THEATRE, Project Stage, Souderton, PA.

Price: $185, $25 sibling discount off of full amount.

This is to certify that the above registrant is in healthy physical condition and does not exhibit any signs of illness from COVID 19 (or variant) virus.  If the registrant is subject to any medical problem please indicate this on the application.  We (I) agree on our (my) behalf and the behalf of our child not to hold Jennifer Wilkinson and/or any instructors responsible or liable for any injury. We (I) agree to release and discharge Jennifer Wilkinson and instructors and Montgomery theatre, from any claim, demand or damages and from any action arising out of any occurrences while at the class/show.  We (I) have insurance coverage which we (I) feel is adequate.  I understand clearly that the organizers of the Little Kicks Theatre/Montgomery Theatre/JenniferWilkinsonMusic LLC, do not carry group medical insurance., Masks will be optional(beginning classes) and social distancing will be in place to provide a safe experience for all.  Guidelines are part of an evolving plan and in accordance with our governing agency and as information and changes are made available, we will adjust our stage policies and procedures as needed.
